About this toolkit
The Dental Home Initiative for Persons with Developmental or Intellectual Disabilities is a 25-page program and grant-development guide based on an oral-health access initiative in Palm Beach County, Florida. It describes how community agencies, dental hygienists, clinicians and caregivers can work together to establish ongoing dental care for people facing access barriers.
What the guide contains
- Identifying oral-health disparities and barriers to receiving care.
- Developing a dental-home network, identifying partners and seeking grant support.
- Planning screenings, preventive fluoride services, education and referrals.
- Training caregivers and community dental teams.
- Tracking program outcomes and using forms, consent templates and supporting tools.
Reported outcomes
The historical program report describes screenings for over 550 clients across nine centers, preventive fluoride varnish applications and caregiver education, and additional provider referrals. These figures describe the initiative reported in the toolkit and should not be interpreted as current service availability.
